Just a reminder as fall sports are coming to and end. If your child is playing basketball they need to be up to date on their physicals.


PHYSICALS (RSU#12 Policy Code: JLCA 12/2011)

Students who wish to participate in middle school or high school athletics are required to have a physical examination and medical approval before doing so. The physical examination must have been given within 2 years (730 days) prior to participation including tryouts. Thereafter, students participating in athletics must have a physical examination on a schedule recommended by their health care provider, but not less frequently than once every two years. A student will not be allowed to participate in tryouts, practice or competition without the required physical examination and medical approval. In the years that a physical examination is not required athletes must submit a health history form which is reviewed by the school nurse.

The school nurse, after reviewing the form, may refer the student to his/her health care provider for more extensive examination. A student whose health status is questionable and has been referred for further examination will not be allowed to participate in practice or competition until he/she has been examined and medically approved for participation.
